The Admissions Office will assist in applying for Medicaid to supplement all primary insurance plans.

Admission criteria for acute rehabilitation program referrals:

• Diagnosis consistent with the program
• Clear rehabilitative outcome potential to return to available community residence
• Patients ability to follow directions/instructions
• Involvement of family/friends /community support system, if any
• Payment source to cover rehab stay
