A new addition to Greater Harlem is the Continuum Hospice Care. Hospice Care is comfort care provided to residents who are terminally ill. These residents get a one on one certified nursing assistant, and a registered nurse who visits daily along with the care provided by the staff in the facility. As the resident benefits from the care provided by caregivers, the families will also be able to benefit from the Round Table for Families at Greater Harlem Nursing Home. The Round Table for Families is a group held every 4th Monday of each month at 5:30 - in the facility under the leadership of Dawn Smyer, Psychologist. This is where the families are able to express themselves about their loved ones, share any experiences that they have had and also provide support to their peers. If you or some one you know would like to talk about managing change, have the need to let go or is reaching out for support, join the Continuum Hospice Care Round Table for Families at GHNH. To register, contact the Nursing Home’s Social Services Department at ext. 263.
