The Multi-Disciplinary Team

In order to ensure the best possible care, each resident is treated by a multi-disciplinary team, made up of staff from various backgrounds that can address the unique needs of each resident.  Upon admission, each resident receives a complete evaluation performed by the physiatrist, who then prescribes an individualized plan of care and rehabilitation.  The multi-disciplinary team includes representation from: medical; social services; recreation/activities; speech-language pathology; occupational and physical therapy; and rehabilitation nursing staff.  The team provides ongoing care throughout the rehabilitation process that cares for each resident, and coordinates the plan of care, treatment and discharge back into the community.
