Physical Therapy
Occupational Therapy After the physician’s initial assessment of the resident, the occupational therapist evaluates how any physical, cognitive, perceptual or psychosocial difficulties may hamper the resident’s ability to function independently at home and in the community. Residents are treated through various activities, which may include self-care education; the use of adaptive equipment; function-based cognitive and perceptual training; and balance and coordination programs. The ultimate goal of occupational therapy interventions is to maximize each resident’s level of function at home and in the community. Rehabilitation Nursing The nursing staff and nursing aides work closely with each resident, the family, and the multi-disciplinary team at every stage of rehabilitation. The nursing staff helps the residents to apply the motor skills that residents learn through physical and occupational therapy. Residents are encouraged to practice and perfect daily activities such as dressing, bathing, eating, and walking. Speech-Language Pathology The speech-language pathologist assesses the impact of any communication or cognitive difficulties on the resident’s ability to function independently. Through individualized programs, the speech-language pathologist also works with individuals who have difficulty with swallowing, the medical term of this being, dysphagia. The speech-language pathologist creates a program and provides a recommendation for each resident regarding appropriate meal time management and safety, compensatory strategies, and follow up care. Residents’ Trust Greater Harlem Nursing Home & Rehabilitation Center has set up a trust fund for its residents. In a partnership with our neighborhood bank, the trust fund was created to allow residents to manage their funds in a safe and secure manner. Deposits to and withdrawals from the trust fund can be made through the nursing home’s Business Office during hours that are reserved solely for this purpose. Greater Harlem also has one staff member dedicated to the management of this trust fund to ensure that monetary transactions can be made easily and securely.
Social Service The Social Service department guides residents and families through the admission and discharges processes and aides them in the transition to life in the nursing home. Social Service also files and manages residents’ Medicare/Medicaid applications, and secures auxiliary services for residents such as visiting nurse services, Access-a-Ride, housing for discharged patients. Upon admission, Social Service conducts a psycho-social assessment and a spiritual assessment to obtain pertinent information about each resident’s social and spiritual needs so that appropriate services can be provided. Family and Friends Group The facility is supported by a very active voluntary Family and Friends Group (past and present) who serve as a liaison between the residents, administrative staff and the public. Pastoral Care Pastoral Care provides religious services which are offered for all faiths and counseling services when it is requested. Beautician/Barber Services The Beauty Parlor and Barber Shop promotes a social atmosphere where residents can relax to music as they get their hair styled.

| HOSPICE CARE |
The new addition to GHNHRC is the Contiuum Hospice Care program. Hospice Care is comfort care provided to residents who are terminally ill. The residents get a one on one certified nursing caregiver, a registered nurse who visits daily along with the care provided by the staff at GHNHRC. As the resident benefits from the care provided by caregivers the families will also be able to benefit from the round table for families at GHNHRC.
