General Information

The Greater Harlem Nursing Home & Rehabilitation Center is a non-discriminatory facility in which no one will be denied admission due to race, color, creed, religion, country of origin, sex, age, handicap, marital status, sexual preference or source of payment as long as they meet the established and approved criteria.

 

The Admissions Office will apply for Medicaid to supplement all primary insurance plans.

 

Admission criteria for acute rehabilitation program referrals:

  • Diagnosis consistent with the program
  • Clear rehabilitative outcome potential to return to available community residence
  • Patients ability to follow directions/instructions
  • Involvement of family/friends /community support system, if any
  •  Payment source to cover rehab stay

HOSPICE CARE
ImageThe new addition to GHNHRC is the Contiuum Hospice Care program. Hospice Care is comfort care provided to residents who are terminally ill. The residents get a one on one certified nursing caregiver, a registered nurse who visits daily along with the care provided by the staff at GHNHRC. As the resident benefits from the care provided by caregivers the families will also be able to benefit from the round table for families at GHNHRC.
